Application Documents
If you are a new client at The Sensory Gym, you will need to complete the Referral Questionnaire, Sensory History Form, and Risk Management Document to apply for an intensive slot. If you are a previous clientat The Sensory Gym, you will only need to complete the Risk Management Document. Please make sure your Adobe Reader is updated to avoid any errors.
Both new and previous clients will also need to have:
A copy of your child's current NDIS plan (if applicable)
A current photo of your child
These will need to be uploaded when you put your name down for your selected intensive so please look at filling these out ahead of time and saving them on your computer or phone ready to be uploaded when the books open. Once you have your documents ready, click the open link for your desired holiday intensive above and complete the application.
